I guess Sid Meier is a Mac fan since I've been unable to reinstall Civ 4 (and all of its expansions) since the Steam update.

Weired indeed. Especially since I actually have them double (one time for Windows and one time for Mac) on my list:

I would open a ticket with Steam Support if I were you and ask them to fix it.

I have them in double too and can't install the "MAC" one.

It's been known since the release of CIV4 for MAC...

Ah, I didn't realize there were duplicates. I had the Windows versions in a minimized category, and can install those. Don't know why it shows up twice, and I have so many games that it's hard to keep track of them all.

Move along, nothing to see here.
