After reading through and experimenting with numerous things, this turned out to be the best.
First, log into steam, either in online or offline mode. Then access the community section. A box checked of for "In-Game Steam Community" or something similar to that. Make sure it's disabled. Afterward, the game should work perfectly. I've been able to jack up the the graphics all the way.
My laptop has Windows 7 and ATI, which seemed to be the problem children with KOTOR initially. This is also confirmed to work on some macs (see thread above) as well as systems with Nividia graphics card, and Windows '98, 2000, and Vista. Thanks to Dipnlik for the initial post.
If anyone has problems with the 59Hz problem, I can help with that too. Just post here or message me and I'll walk you through it myself, or direct you to some steps that can help you.
