How did he get up there?

This used to happen at the top of the ladder to the roof in NM (Finale) in L4D1, where there was no railings.

Old Smokey could step into thin air and would "walk" back whilst choking a Surv.

the surviver was flying upwards and I smoked him so he did not move anymore.
