You really should provide more information, so you can get the best advice.

Have you verified your local files?

Does your settings include V Sync on or off?

Does this happen on every map?

Does it happen after a short while or immediately?

OK: what are your system specs?

(To save it being one question at a time, try and give as much information as possible)

Win Vista Sp1
4GB Ram
Intel Core 2 Quad 2,66 GHZ
Geforce 9800 GTX/9800 GTX +
32 bit

i played it before normally but my win failed and i must downloaded l4d 2 again and this happened.

I tried Verified Local Files but it still same.

so did you backup your other L4D2 files, install your windows operating system and then re-installed L4D2 from your backup?

If so, at what point did you verify your files?

This is screen tear and usually associated with non v-synced settings

Change your v sync to double buffered and off. Try both in single player and see what occurs.

EDIT: Also get something like MSI Afterburner (click for download) (http://downloads.guru3d.com/MSI-AfterBurner-download-2392.html#download) and check your temperatures, just in case it's a hot gfx card.
