I start the game, the start menu screen is different
But I found both start in same place...
(What's the point of TQ stand alone then?)
And IT have some import function...em...
Do I "miss" something if I play in TQ?
Or should I ignore TA and play TQ:IT
TL;DR. Can I install only TQ:IT and played all content?

You can't install TQ:IT expansion without the original Titan Quest installed.
You should launch the game with the TQ:IT shorcut to get all the new features of the expansion.
The beginning of the game is the same.

Difference between the two:
The original Titan Quest simply lacks the new Dream mastery, extra 'campaign' along with shared character stash, new weapons, new armor + recipes/relics and other general improvements Immortal Throne introduced to the game.
So yeah, there's no real point in playing the original game over IT if you have it. :)

if you are starting from scratch
Download the game TQ + TQIT, update through Steam
Start up TQ and create a character
Exit
Start up TQIT and import your character
edit:
You only have to do this for your very first character.
All subsequent characters are started in TQIT itself
Playing the game in TQIT gives access to the new Dream mastery, Hades level, new loot.

Hm, I'm curious, I had TQ + TQIT but didn't played it yet, but plan to start soon, I hope. Why you needed to create character in TQ? Why just not start TQIT and create character there, is there reason to start character in TQ?

TQIT is an expansion pack and requires the presence of Titan Quest Files to run correctly.
Creating the first character in Titan Quest and then importing it to TQIT "primes" the game.
You only have to do this for your very first character.
All subsequent characters are started in TQIT itself
