Sounds like packet loss from a bad connection to the server. Type net_graph 1 into the console, this will tell you your framerate, ping, loss and choke. Anything over 5 in loss is bad, over 10 and the game starts to become unplayable. Sometimes it's just a problem at the server end, but if it's happening all the time and no-one else in the server is experiencing it then it is a problem with your connection. Monitor these values when you play and complain to your isp if it persists. BTW don't play on wifi, that will also cause this problem.

Try turning off multi core rendering

Multicore rendering causing uneven framerates would also effect single player. OP says it only happens in online games.
