Is there a list of gamepad controls anywhere? Couldn't find one in game.

When you choose configuration pre-launch there is a tab for 'Keymap'. If you're using a controller, then choose the Xbox 360 Controller option in the lower left (should say Generic PC in default). After that it's fairly straight forward.

When a symbol appears on the screen when i fight monsters what key do i have to press(360 controller),i press B to start the attack but then what? i always fail.

Yes, it's not clear at all, but it's actually a direction. So, you just tilt up, down, left, right according to the icon on the screen.

I figured that out on the second go, because the grab button and the QTE on the same side were impossible to input at the same time.
