I think it increases the chance of key/bomb/heart drops after you clear a room.

As far as I know, the pills have a much higher chance to have a positive effect.

It increases the rate of finding items after finishing a room, or just finding rooms with chests in them. It has nothing to do with pills, the PHD actually does that.

I think it also raises the max hp by one (cain is described as havng only one heart but he starts with 2)

Judas character has 0 in hearts stat but starts off with 1 heart

I seemed to get more coins with it but I could be wrong. For instance the machines gave sometimes 3 coins when other characters only got 2 at the most for me and not often. All speculation but not something easy to test.

I noticed that on my first Cain playthrough EVERY single pill had a positive effect, so its possible it has the powers of PHD as well.

I definitely got bad pill effects with him so this is not true.
