can you keep an item equipped?

for example, i found moms heels, then in the next basement i found moms underwear (might be the other way around). is there a way to choose which i keep equipped or something?

They're both still "equipped" you just don't see the heels anymore; you still get the stat bonus from both of them.

If you mean can you choose which one shows, you can't. It's whatever one you picked up last.

thank you for the fast response. also, how do i save? or since its kind of rogue-like esque, is there no saving?

No saving, but it should keep track of your progress in stats and collection. As you progress further, more stuff will be shuffled into the randomness.
