Does anyone have the same issue?

I've created it several times, and the image seems to be missing.

I don't have image even in steam folder so i thing that everyone missing a icon image

The picture for the icon can be found in steam/steamapps/common/cave story+/data

Properties on the shortcut, go to that folder, select icon.bmp, should work :)

Missing icons is something that happens to many games in Steam. Don't know why really.
