No visual at all when game starts.

When I start the game, the screen goes black and the theme plays, but I don't get any visuals, except for a tiny square cursor that appears when I move the mouse or press buttons.
Is anyone else having this problem?
My graphics card is somewhat old, as I got this computer in 2007, but I find it hard to believe it would crap out on me for this game, especially when I've never had any problem running the game outside of Steam, in its previous, non "+" incarnation.

It happens to me randomly. I start the game and suddenly I hear the music and realize nothing showed up. Sometimes it does it and sometimes it lets me play.
