I'm not sure if it matters if you haven't built up any combo when you get hit/die, but if it breaks your combo, yes. Probably even if it doesn't, but I haven't checked.

ya you need to chain the entire levels worth of dirt in order to get an S rank. If you get hit (and you have started the combo) then it will instantly deny you of that S rank. Also if you wait too long, and your combo drops you wont get S rank either.

To sum it up, any time your combo ends and resets you've lost your S for finesses
