There was a problem adding 'name' to your friends list

I'm trying to add friends and I keep getting this error:

"There was a problem adding 'name' to your friends list"

What could be wrong? I've asked the friend to add me and they get the same error.

For some reason I've had some friends in 'invite' status for months now and whenever I'm online they tell me they can't see me at all. What's going on?

Friends in Steam.
• Did you try the action once? More than once?

I added “arnieonthebeach” Friends tells me he is invited but NOT online.
Actually he is online and connected to Steam!
Same at his side. He invited me and gets the message I’m offline which is not correct either.
I removed him from Friends and try to add him again. Now I get:
"There was a problem adding “arnieonthebeach” to your friends list"

That applies when you try to add a known generic name; John or Jane for example. Especially playing games, people tend to use the mini Friends client instead of the Search function.
