Sometimes when your king dies you lose all your provinces?
How is that possible? It's really annoying!

Usually it has to do with your marriage alliances, others lay claim to your kingdom.

If you do not have a proper heir (sorry, in this game only male heirs may ascend to the throne alone), then it is possible the different nobles of your many towns will decide to rebel and create their own city-states (which may later grow into Kingdoms with multiple territories).
I once did this on purpose just to make the game more difficult.. I was Pagan Lithuania, and I converted to Catholicism, and I lost ALL of my territories except Samogita and Crete (Crete because I had marshals nearby to re-claim it). It was a very exhilarating (for this game) experience.

It only happens, so far i know when your kind dies and there is still some nostalgia in the provinces you lose them.
