You Only Live Thrice achievement glitched?

I defeated a skeleton or zombie, forgot which. It re-animated, so I killed it again. It re-animated again, so once again I put it down and won the fight. The achievement didn't unlock. I've defeated a number of re-animating monsters and this achievement has yet to unlock. Is it glitched?

A random skeleton finally gave me the achievement.

i think it has to reanimate 3 times total, not just two. i've had them reanimate twice several times and haven't got the achievement myself, but never seen one do it 3 times
