I have A, W, and M achievements but not BT

I'm fairly certain you have to get BT to get A, W, and M.

I know I played this game a bit offline. Is it because I got what would have been the BT achievement offline, that's why it doesn't register?

Try to redo the first two levels after finishing the game.

Yeah you should already have that achievement. You can get it by beating the first couple levels on normal difficulty.
