Normally I'd hate to necropost, but I had this same problem on the PC and found a solution that doesn't require a bunch of trickery.

First off, you don't need to get the MAVI scan in the same run, only the two documents on the body. As for how to get it, you have to bypass it entirely first, and get to the cutscene at the top of the shaft that gives you the gun. Break the glass windows to flood the room, then open the door to flood the shaft. Simply dive in, grab the documents off the body, and swim back up. Medal gets unlocked.

Reinstalled to give this a quick try and still hasn't unlocked for me, this game will forever taunt me with that last achievement.
