Motb: Headmistress Tower

In my journal it says I have all four souls, but when I go to the door I can only put in three. Anyone know a way to fix this? I can't find the fourth soul in my inventory when I look but I know I did all the challanges.

I suggest you post your question in the MotB forums here: http://social.bioware.com/forum/1/category/170/index. Chances are greater that someone reading those might know a work-around.
