I don't quite get the whole thing you want. To add a Sandvich to the Shotgun while trading a Minigun away?

Basically, M2 on all Heavy secondaries gives a medium health kit, but you lose your secondary until it recharges. Shotgun shoots stuff with m1. Sandvich gives 300 health back with m1.

Better yet, why don't we let Heavies have both be equipped at the same time.

Slot 1: Minigun
Slot 2: Shotgun
Slot 3: Edible
Slot 4: Melee

Because more slots solves issues.
