Keybind problem / no character response

I've been playing this game for years, but two issues pop up out of absolutely nowhere.

1. The "play with controller" option is set to off, but when I get into the game I can literally do NOTHING. No moving, no turning, no shooting.

2. When I try to click on a keybind in options to change it, it automatically binds it to LeftMouse. It doesn't even give me the chance to put a different key in.

Any help would be appreciated.
