You're talking about 3rd party servers, where server owners can do what they want. If you want to make sure you don't play with them, only join lobbies with a leader you know or get your own server.

To be honest, people like this make me laugh rather than get annoyed. They're clearly terribly angry children and as long as you just laugh in their faces when they resort to blatant cheating, you'll come out on top (whether you technically win the game or not).

Yeah but unfortunately 3rd party servers are the only servers that give me decent ping. The Official Dedicated Servers are hard to come by and bad ping all round.

Laughing in their faces is the best cure, because it causes them to reveal what they were doing all along. I usually get asked how i 'knew' after the reveal. It is funny when a horde of tanks, witches suddenly spawn right in front of you, an airliner drops from the sky on you and you're suddenly back in lobby...lol

I want to add to what MilkyCookie has said.
Many nowadays cheaters are using NoSpread hacks, they might be aiming manually by hand, but NoSpread hack allows them to kill SI on greater ranges and while swarmed by horde.
Also cheaters can always kill Smoker while being smoked with NoSpread hack. Normal players will have hard time doing this, because their spread is increased while they are smoked.
NoSpread hacks are not always make screen shake while normal player spectates cheater in game, despite some opinions about that.

Many cheaters are using edited(for lower volume or higher volume) sound files or even deleting some of them, to hear more than others. They seems to be always prepared and acting instantly. Normal players can't hear a Smoker spawned nearby while every one around them are shooting and they are swarmed by horde. Headphones doesn't gives them an ability to skip unimportant sounds and hear only important ones. Headphonea also doesn't give an ability to shoot preciselly by sound, by they can try to shoot only in direction of that sound.
MatHax and Sound cheaters should be dealed with by Valve, when they patch their SRCDS(Source Dedicated Server) and release Steam Workshop for Left4Dead 2.

Hackers also use various hooks to game's events, like:
"jockey is about to jump on you" - changing jockey's jumping way a little, or just bash him, if fatigue level allows it. Normally undetectable, because it's already hard to jump on a moving player with jockey.
"you are about to be smoked by Smoker" - After Smoker is launched their tongue, they extremely fast switch to a melee and cut tongue. Normally you can cut a tongue with some melee weapons, but you can't switch to melee that fast and most of the time you have to be prepared( as I said above, it's not easy to hear "Smoker spawned" and "Smoker attacked" sounds).
"Hitting undamaged Charger in a head with melee" - Again it's incredibly hard to hit anything precisely while being swarmed by common undead, but Event-hooking allows cheaters to magicaly "hit" right in a head while being swarmed. Normal players can do this, but chances for success are not worth this, and ofcause they can't do this while being swarmed by commons, apart from plain luck. Partially damaged Charger can be Leveled without hitting in it's head, so Leveling a Charger can be accomplished much easier, but there is always chance to fail because of network latency as it is for every action in game.

As a word of advice, practice with friends and see if some tricks you've seen in games are possible or not. Discover limits of the game and you will be better at spotting cheaters. Watch demos of a cheaters recorded by spectating players.
Also play only on:
* VAC Secured servers (can be checked with "status" console command).
* Servers with SMAC(SourceMod Anti-Cheat) installed.(can be checked with "sm plugins list" console command)
* Check game settings with "differences" console command. It will show what CVars were changed from default values by client settings, and also it will show what CVars were changed by server(replicated ones), when your team play as Survivors, or Infected. This way you can spot admin-cheat servers.
* Create lobby and set trusted favorite servers with "mm_dedicated_force_servers" console command.

Always block cheaters, record demos and tell others about them, so not you nor other players will play with them or against them.
As a last word, it is sad that even confogl mix lobbies are not 100% insurance against cheaters.

I'm not that experienced at L4D2 but I can't imagine it's that hard to melee a charger at the right time while horded. Also, I can definitely distinguish SI sounds while horded.

Yes, I know many players those are better at game than me, and I respect that.
Most of the cheats are undetectable and can't be proved in game. Watching recorded demos after, especially those were recorded by spectators can be an eyes opener.

Yes, you could hear them spawn, or see "Close Captions"(which also considered a cheat), but you can't pin point their exact location only guess judging from your experience. And you can't react to them right away and aim properly while commons hitting you, because your spread increases and crosshair moves.
Cheaters with NoSpread are ignoring commons and always shoot preciselly in center of crosshair, this can be seen in demos.

I've only ever seen one cheater although he could be a vet. He had all of his kills as headshots and he killed around 400 infected.

Yeah there are a lot of cheaters lately. Always knows where you would spawn, following our ghosts around and when we coordinate he just 1 shots us on the head and quickly snaps to the other SI and kills all of us just like that.

But once we wipe them out and doubles our lead, they quickly ragequits ^_^. Yeah even when they are cheating, win or loose they pretty much are losers.

Edit:
Why is Vac not updating their detection system? We searched around the net and there are cheats that said it's VAC3 undetected? People actually pay money to cheat. Pls Valve reverse engineer this cheats and update the detection system often.

That is what they do, people report cheats and vac updates itself with them.

Members of the VAC team also search the internet for cheats to ban. I was told this in aa group chatroom so I can't post proof but it makes sense that they would do that.
