where is he? i have cleared stench trench and been around the map corner to corner several times and i can't find him...

I have the same problem. Have cleared the level but didn't find him or any elevators to lower levels.
Is there something I have missed?

He is there somewhere, I had the same problem first time I went there. I was so busy fighting that I didn't notice the name over his head so I didnn't notice it was him I fought.
He will respawn if you exit the game and reenter it...bad part is you have to walk all the way down to kill and loot him again.
