I have found the following posted on other forums.

Problem #5 : Call of Duty Modern Warfare 2 IW4SP.EXE error / “No IW4SP.EXE error”
Solution : In the game list in steam, right click on the Call of Duty Modern Warfare 2 then click “Verify Game Cache”

I have just done this with my instal and it did indeed find a nackered file, ill retry.

The following was also found and lets face it anythings worth a try

Problem #3 : Call of Duty 4 Modern Warfare 2 Crashing / Freezing ingame / IW4SP.EXE error / “No IW4SP.EXE error”

Solution #1 : just disable Steam community in-game.
credits: icc

Solution #2 : just go in Steam\SteamApps\common\call of duty modern warfare 2\Redist\DirectX and run DXSETUP.
credits: Fulger

Solution #3 :Set your desktop to a resolution of 1024×768, 32 bit True Color.

None of the above worked for me.

Yes, I did last night. With the 191 drivers, I hit one DirectX crash and one sound-related crash. I haven't had any problems with the 195 drivers so far. They seem to be a bit more stable.

Give these a shot if you have any DirectX issues.

e.g. You hear the infinity ward logo spin around but no graphics appear at the start of the game. After I updated the drivers, I see this logo now.

Ok installed a ATI 4870 last night in the same rig, and NO issues at all, put back in the Nvidia card and it crashed within 15 mins, checked the sys logs and its related to nvd3dum.dll, looks like its an issue with the Nvidia CP/Desktop manager.
Is any one getting this issue on any ATI cards?

Agreed this needs sorting, or even some official recognition of the issue by IW/Activision, but i suppose there interest will wane now they already have my hard earned cash

POSSIBLE RESOLUTION - WORKED FOR ME
I found a way around this.
Its the Video options in the game that were causing it for me. It only works if I leave AA on as 2x (default), but can pretty much change everything else.

Now no crashing, and luckily my gfx is powerful enough to play 1920x1200 with 2xAA

iw4mp

I'm running win7 64bit RC latest nvidia beta driver and had this resetting audio and video back to optimal settings sorted it for me seems like IW don't want us to tweak our settings to improve our fps
