Black ops hacker "Unknown Soldier"

Yet another blops fail. Hackers are able to disguise who they are, appearing as "unknown soldier", so they can't be in any way caught and probably can't be banned from the server.

I guess setting up the servers so that "unknown" players couldn't join would be too much trouble, eh?

^^ This

You can constantly change your profile name, which is the name that appears when you are playing BO, if it banned you via an in game name just think how damaging that would be, some hacker has it in for a clan and cheats using the name of everyone it getting a whole clan banned.

Each game has it's own unique GUID, related to the serial number issued for that copy, this is what gets banned
