xbox 360 gamepad button mixup after update

Hi everyone
After the update I started up skyrim could use the main menu with the A button but no B for back, went in game and I've got a custom button config (a-jump, b-put away weapons, X-activate and y-menu) everything works while inning the game but go into a menu and controls are mucked up a, does nothing, x does nothing , only b does anything and it allows me to drop items.

So I set everything to default and its fine, re do my custom config for the buttons and the same thing happens.

Oh and keyboard controls are fine but I prefer to sit back with the pad.

Any ideas or do I just play with keyboard till its fixed.
