Atronach Forge not working

I have tried correct recipes and nothing.

my conjuration is 100, main level is 41, race is Orc.

I did try:

Sigil Stone
1 Human Heart
1 Black Soul Gem

then won't spawn an item, is that bug?

Need a help

Hmm. You did put the Sigil Stone in the setting instead of the offering box, right?
