Any tips for getting this achievement? Although it really seems to just require lots of luck, since as far as I know only 2 weapons' ammunition can malfunction and the chance is 5%...

Yeah, it's pretty luck based. The malfunction-prone weapons do have much higher malfunction rates at the lower levels. Once they're fully upgraded, they malfunction pretty rarely. So, if you're chasing that achievement, one thing you can do is start a new game and try to get it with the beginning level weapons. I'd back up your save game first though... make a copy of "My Documents\Xotic\Campaign Progress.dat" so you can restore it after getting the achievement. Hope that helps!

I just got lucky and got this achievment 2 days ago. I´ve made nearly a whole run of the game leaving the 2 malfunctioning weapons without upgrades and tried to give (when possible) the finishing blows with them...which makes some battles pretty hard by the way
