Possible bug to laws in latest patch?

I've been experiencing a few seeming failures of the legal system since the latest patch:

Current laws are Agnatic Primogeniture with Medium Crown Authority

1. Vassal wars. My vassals are still fighting each other for territory.

2. When my son died of a deluge of mental illnesses, the system seemed confused over which grandson should inherit and kept toggling between them for a few years before finally correctly settling on the elder grandson.

3. When the younger grandson violated the Crown Authority yets again and launched an ill-fated bid for his elder brother's lands, he was captured and executed as a traitor. The system then suddenly decided that HIS just-born son (great grandson) would be my heir.

What the deuce is wrong?
